Finally, engagement in a final family interview to discuss educational plans and placement recommendations. 1. Appointment Process:

- Schedule an appointment for a family interview with the school administrator at (352) 255-7255.

- Interview includes a family tour of the school and discussions.

- Prospective students must be present.

- Required documents (refer to the Documents tab):

- Birth certificate.

- Immunization and physical forms.

- Any 504 plans and IEPs.

- Any Psychological evaluations.

- Parent/Guardian ID: Valid FL Driver’s License with Current Physical Address, Photo ID or FL Voter’s Registration Card

- Guardian: Court Custody Documentation

2. Assessment Procedure:

- After the interview, an assessment test for your student will be arranged.

- Allow the following estimated times for the assessment:

• Kindergarten: 31 minutes

• 1st-3rd Grade: 66 minutes

• 4th-6th Grade: 112 minutes

• 7th-8th Grade: 127 minutes

- Timing depends on the student’s ability and speed.

- Assessment takes place at the school; parents can wait.

- Results available within 3 business days.

3. Final Family Interview:

The office will call to schedule a final family interview.

- Student must attend.

- Review and discuss student assessment.

- Determine educational level, plan, and placement recommendations.
